  1. Recognizing the transition from chronic complex illnesses to chronic progressive illnesses
  2. Identifying when to have conversations about limited life expectancy and the topic domains where clinicians need to  initiate the discussion
  3. Recognizing the enormity of care offered by family and friends in the last year of life, and how we can better support these people while in the role and subsequently
