Club Sports Enrollment Verifications

Student club sports teams may require enrollment verification to participate in tournaments, races, championships, etc. Follow the steps below for verification support.

Process

1. The team's captain downloads the Dartmouth's Club Sports Verification Form and works with teammates to complete the form. Signatures from all teammates are required to proceed.

2. Submit both the Dartmouth Club Sports Verification Form and any third-party verification form to the Registrar's Office at least one week prior to the due date.

3. The Registrar's Office will forward the completed form to the third-party directly and will notify the team captain once the form is sent.
