Eligibility
Any employee, adult dependent, or retiree enrolled in a Dartmouth medical plan (without a Health Savings Account) is eligible for One Medical at Dartmouth.
- All benefit eligible employees and their adult family members (18+) enrolled in the OAP, CCF, or HDHP with HRA medical plan are eligible to use One Medical at Dartmouth, if you wish.
- Employees who waive medical coverage or who are enrolled in the HDHP with HSA are not eligible to use One Medical at Dartmouth.
- Retirees who are enrolled in a Dartmouth medical plan are eligible to use One Medical at Dartmouth, if you wish.
Please note: you do not have to use the One Medical at Dartmouth practice, and you may change your primary care provider at any point during the year.
How to Enroll
One Medical at Dartmouth is accepting new patients! Here's how to get started:
- Enroll at onemedical.com/dartmouth
- Click on Activate Membership
- Employees, use your Dartmouth NetID
- Spouses/adult dependents, use the employee's NetID; it will then prompt you for an Activation Code, which is DMCXOM
- Retirees - please call 603.738.1164 to register and provide activation code DMCXOM
- Follow the registration prompts
- Once you've registered, download the free One Medical app via the Apple App Store or Google Play

How to Disenroll
If you wish to leave the One Medical at Dartmouth practice, please follow these two steps:
- Log into the One Medical app, click on My Health, select Get My Records, and follow the prompts to have your medical records transferred to your new provider.
- Email dartmouth@onemedical.com with your name and date of birth and ask to deactivate your account.

History
Dartmouth launched this primary care practice in 2012 in partnership with Iora Health as a convenient way for employees to access high quality, innovative care. Iora was acquired by One Medical in 2021, which was subsequently acquired by Amazon. Although ownership of the practice has changed, One Medical remains committed to providing the excellent health care and personalized patient experience that has been Dartmouth's goal from the beginning.
