Dependent & Event Verification

Dependent Required Documentation

When adding a new dependent to medical or dental coverage, the following documentation will be required.

Spouse

  • Marriage Certificate (from town or city)

Child

  • Birth Certificate
  • Hospital Issued Certificate
  • Court Documents (Proof of adoption or legal guardianship)

Event Required Documentation

When completing a qualified life event, you may be required to provide proof of the event.

New Hire Event - Required

  • Adding a Spouse or Dependent Child please refer to Dependent Required Documentation above

Marriage - Required

  • Marriage Certificate (from town or city)

Birth/Adoption/Placement for Adoption - Required

  • Birth Certificate
  • Hospital Issued Certificate
  • Court Documents (Proof of adoption or legal guardianship)
  • State issued Parent Notice

Divorce

  • No documentation required

Employee and/or Dependent Loses Eligibility for Outside Coverage - Required

  • Coverage Cancellation Notice
  • Confirmation Statement that Includes End Dates of Outside Coverage

Employee and/or Dependent Gains Eligibility for Outside Coverage - Required

  • Copy of Insurance Card that includes coverage start date
  • Confirmation Statement that Includes Start Dates of Outside Coverage
  • Document Showing Other Coverage with Start Dates

Health Savings Account

  • No documentation Required

Open Enrollment

  • Documentation is not required for the event, only when adding new dependents
  • Adding a new spouse or dependent, please refer to Dependent Required Documentation above
