Winter Break Incident Reporting

The Office of Risk Management & Insurance will be closed during Winter Break, Saturday, December 21 - Sunday, January 5.  

While we are always hopeful that no injuries occur, we ask that you follow these important steps to report any incidents during the winter break.

Winter Break Incident Reporting Procedures: 

  1. Complete the injury report form within 24 hours of the injury, following normal procedures found on the Reporting Employee Injuries site. The form can also be found on this page.  
  2. Email the completed forms to both Lisa Roche (Lisa.A.Roche@dartmouth.edu) and Risk.Management@dartmouth.edu. It is crucial that the report is sent to both Lisa and the risk management team.
  3. During Winter Break, please call the office at (603) 646-2442 as this line is monitored. 
    👉🏽Please call immediately for any serious injuries or accidents (fatality, amputation, vision loss, immediate hospitalization). If you are on the fence on whether to call, please do so.

Email will be monitored during the break to ensure timely reporting of injuries to the State as required. The office will re-open on January 6, 2025, at which time normal reporting procedures will resume.
